Primary care plays a critical role in the management of diabetes, as early detection and consistent monitoring are key to controlling the condition. Primary care providers (PCPs) are often the first to identify diabetes through routine check-ups and screenings. They work closely with patients to manage blood sugar levels, recommend lifestyle changes, and prescribe medications. Regular follow-ups help track progress and adjust treatment plans, ensuring personalized care. PCPs also educate patients on self-management, including diet, exercise, and blood glucose monitoring, to prevent complications like heart disease or neuropathy. Collaborative care, involving dietitians, endocrinologists, and educators, enhances overall diabetes management and improves long-term outcomes.
